Spring Track Sports Participation
Cost: $200
All registrations must include: (1) the athletic registration form [pdf, Word], (2) waiver form [pdf, Word] (3) current physical (within the past year)[pdf, Word] and current medical history [pdf, Word] , (4) emergency card [pdf, Word] . NOTE: If you have previously registered for a sport and have been cleared, only the registration form w/ check attached is necessary.
